Resolution: We improved the existing "File Director for Windows" by developing an iPad app for immediate access and enhanced security.

Problem: Patient's risk for hospitalization can be time-consuming and susceptible to human error.

Goal: Save clinician's time by surfacing critical trends. Reduces hospitalization rates by enabling proactive interventions.

Solution: Alert clinicians to actionable trends without disrupting their workflow.
